Health Care Home Ernst and Young evaluation

We are delighted to publish the recent evaluation of our Health Care Programme undertaken by Ernst and Young. The results are really exciting and highlight the benefits to practices, their patients and the broader health system of the Health Care Home approach.

The Health Care Home is also no longer just a Pinnacle activity, there are now nearly 130 practices nationally in Northland, Auckland, MidCentral, Wellington, Nelson, Canterbury and Southland DHBs.

A national Health Care Home Collaborative has been formed that includes district health boards, the Ministry of Health and the GP College as members who are focused on supporting practices across New Zealand make the shift to the Health Care Home.

From a Pinnacle perspective it's really rewarding to see how broadly the model has been adopted, how it continues to develop, and the growing support it is receiving from district health boards and the Ministry.
